Blood on My Hands
I received a notification that someone had been cut in half for a system I designed years ago. I was sick to my stomach. When I learned what had happened, I was disappointed. The man willfully bypassed multiple safety devices, and super-managed the system as it ran.
It was determined that I deliberately circumvented a system which is otherwise safe, since all my risk assessments were in order and risks were properly mitigated as per ISO 13849. It was not pleasant to know that the work I did had essentially taken someone’s life.
