Triple Tragedy
Three people lost their lives at one site. Two deaths in confined spaces and one fall. It happened in Idaho in 2008, when we were working on hazmat for an acid plant turnaround. Asphyxiation was the cause of death in confined spaces. One man went into a tank just to see how dirty and collapsed. What happened to the second guy was even more tragic.
He went into the tank to save the first man, but was not able to get out. They were not allowed to enter the tank, but went anyway. The third fell off the top of the platform, where the safety rails were removed. The sound of the impact was unforgettable. He had taken a 70-foot head.
